Images upload with odd names

This is a strange one, and I don't seem to be able to duplicate it consistently. Sometimes, when uploading an image, instead of naming the uploaded file after the original file (e.g. "abc.jpg" => "abc.jpeg"), it'll name it after the name of the page I'm uploading it to (e.g. "Example.Page.Name.jpeg").
This is quite frustrating, because it seems to keep doing this on repeated tries, meaning that I can't upload a second image to the page (it tells me that it's been uploaded with "Example.Page.Name.jpeg" again, but that name still points to the first image).
It has the curious knock-on effect that the *ImageGallery page shows images that are linked to, but due to the mess-up with the image name, it doesn't link to the referring page.
